As a Physician Assistant focused on wound care and chronic disease management , you'll become a trusted clinical resource across the communities you serve. Rather than simply treating symptoms, you'll help identify problems early, guide treatment plans, coordinate care, and support better outcomes for medically complex patients.

This role combines primary care, advanced wound management, geriatric medicine, and chronic disease oversight into one highly rewarding opportunity.

Responsibilities
  • Provide on-site rounding, primary care, and advanced wound care services.
  • Manage pressure injuries, diabetic ulcers, post-surgical wounds, and complex chronic conditions.
  • Drive chronic care management, preventative care, and clinical coordination efforts.
  • Collaborate with physicians, nurses, therapists, caregivers, and facility staff.
  • Help reduce avoidable hospitalizations through proactive intervention and ongoing monitoring.
  • Communicate with families, caregivers, and interdisciplinary teams regarding care plans and treatment goals.
  • Complete documentation, medication management, diagnostics, and lab coordination.
Qualifications
  • Active Minnesota Physician Assistant (PA-C) license in good standing.
  • Experience in wound care, geriatrics, primary care, internal medicine, family medicine, or post-acute settings preferred.
  • Strong clinical judgment and confidence managing medically complex patient populations.
  • Valid driver's license and reliable transportation.
  • Commitment to patient-centered, outcomes-driven senior care.
