Location Must reside in one of the following Illinois ZIP codes: 60607, 60608, 60612, 60622, 60623, 60624, 60639, 60642, 60644, 60647, 60651. Responsibilities Manage a full caseload of waiver members. Travel 50โ75% of the time to meet members faceโtoโface and conduct inโperson home visits. Facilitate appropriate healthcare outcomes for waiver/LTSS members through care coordination, support, and education. Ensure compliance with contractual requirements. Utilize electronic health record systems and Microsoft Office tools. Qualifications Minimum of two (2) years of case management experience. Proficient with Microsoft Office and electronic health record systems. Reliable transportation and willingness to travel up to 75% of the time; mileage reimbursed. Valid Illinois driver's license. Preferred Qualifications Case management and discharge planning experience. Managed Care experience. Experience working with HIV and AIDS populations. Multilingual language proficiency. Education Bachelor's degree in a humanโservices field (e.g., sociology, special education, rehabilitation counseling) or a nonโlicensed individual with a master's degree in a similar field. Work Schedule Full time, 40 hours per week. Compensation Pay range: $21.10 โ $44.99 per hour or equivalent annual salary. Compensation is based on experience, education, geography, and other factors. The position is eligible for a CVS Health bonus, commission, or shortโterm incentive program.
