Job description

Family Team Facilitator
Department: Juvenile Justice
Locations: Chicago, IL 60616 and 60612
Schedule: MondayFriday, 8 hours per day
Salary: $45,000$48,000 (based on experience and qualifications)
Temporary position with the possibility of permanent employment

Youth Outreach Services is seeking a Family Team Facilitator to support youth who are dually involved in the Juvenile Justice system. This role works closely with youth, families, and formal and informal supports to identify individualized needs and develop coordinated plans of care that promote stability and long-term success.

The Family Team Facilitator collaborates with families and community partners to reduce system involvement and strengthen family functioning through a wraparound, strength-based approach.

Primary Responsibilities:

  • Facilitate family team meetings utilizing a wraparound model
  • Complete strength-based assessments across life domains
  • Develop and coordinate individualized service plans
  • Engage youth, caregivers, and community partners, including child welfare caseworkers
  • Advocate for youth and families to ensure appropriate services and supports
  • Maintain accurate documentation and enter required data for progress tracking and quality assurance

Qualifications:

  • Bachelor's degree required, or 24 years of experience working with high-risk youth in the juvenile justice system
  • YASI (Youth Assessment Screening Instrument) training preferred
  • Minimum of two years of experience coordinating care or providing direct services to youth and families
  • Ability to work varying hours, including evenings or weekends, based on family needs
  • Strong interpersonal skills and ability to engage youth and families effectively
  • Strong organizational skills and attention to detail
