What You'll Do

  • Coordinate client care services, including assessments, care planning, client visits, and caregiver introductions
  • Supervise and support caregivers through coaching, orientation, quality assurance visits, and performance management
  • Serve as a primary point of contact for clients, families, caregivers, and referral partners
  • Collaborate with the Staffing Coordinator to ensure appropriate caregiver-client matches
  • Maintain accurate documentation and support compliance requirements, including long-term care insurance documentation
  • Participate in an on-call rotation to address urgent client and caregiver needs
  • Work collaboratively across departments and perform other duties as assigned to support the success of our clients and team
Requirements:

What You'll Bring

  • Minimum of 2 years of experience in a care-related field
  • Previous supervisory or leadership experience
  • Strong communication, relationship-building, and problem-solving skills
  • Ability to lead, coach, and hold team members accountable
  • Valid driver's license, proof of insurance, and ability to travel locally
  • Basic computer proficiency, including Microsoft Office and data entry
  • Ability to successfully pass a criminal background check

Schedule

  • Full-time position
  • Monday–Friday, 8:30a.m. – 5:00 p.m.
  • Participation in a shared evening and weekend on-call rotation
  • Some flexibility required based on client needs
