Job Summary:
MedStar Health is looking for a Patient Navigator/Community Health Educator to join our team!
Responsible for coordination and facilitating access to health-related services for clients by utilizing available resources.

Primary Duties:
• Supports client care coordination by conducting needs assessments, identifying eligible individuals, and facilitating access to appropriate health services and resources.
• Maintains accurate and timely documentation in EMR and tracking systems to monitor client interactions, services provided, and outcomes.
• Promotes health education, self-management, and quality improvement initiatives through client engagement, counseling, and support.
• Collaborates with internal teams, external partners, and community agencies to ensure continuity of care and address care gaps.
• Ensures compliance with organizational policies, safety standards, and regulatory requirements while actively participating in trainings, meetings, and grant-related activities.
Qualifications:
• Bachelor's degree (Required)
• Master's degree in public health or related field (Preferred)
• 1-2 years in a healthcare environment. Strong understanding of cultural competency with target population
License/Certifications:
• No special certification, registration, or license (Required)
