Facility in Barlow, OR seeking In-Home Primary Care NP locums coverage Rates: $110/hr Opportunity Highlights
  • Contract Term: 13-week initial assignment with options to extend.
  • Guaranteed Pay: Paid for a full day regardless of patient volume (provided availability is logged in Salesforce).
  • Schedule Flexibility: Full-Time: 40 hours/week (Mon Fri, 8 am 5 pm).
    • Part-Time: 20 hours/week options available.
  • No Call/Weekends: Enjoy a stable, daytime schedule with a 1-hour lunch break.
  • Clinical Mix: 70% Telehealth / 30% In-Home visits.
  • Support: Travel mileage is reimbursed at the current IRS standard rate.
Clinical Role & Responsibilities This is an assessment-only role focused on preventive care and chronic disease identification.
  • Daily Volume: Conduct 5 6 comprehensive assessments per day.
  • Focus Areas: Perform annual risk assessments, identify chronic conditions, and review patient health history.
  • Diagnostics: Perform A1C testing only (if applicable); no other screenings or invasive procedures required.
  • Care Coordination: Identify and escalate urgent clinical findings directly to the patient s Primary Care Provider (PCP).
  • Prescribing: None. This role does not require prescribing medications or managing active treatment plans.
Provider Requirements
  • Board Certification: BC Family Nurse Practitioner (FNP) (Required).
  • Licensure: Active Oregon (OR) Medical License (Required).
  • Medicaid Enrollment: Must be currently enrolled/credentialed in Medicaid. Please confirm this status in your submission.
  • Geography: Please specify interest in Lane, Multnomah, or Washington County at the time of submission.
Compensation & Logistics
  • Orientation: 1 3 hours of paid orientation; additional modules are completed during paid downtime.
