Job description

The pharmacist dispenses prescribed drugs in a healthcare facility environment, transfers prescriptions, provides drug information in collaboration to other healthcare professionals, performs medication order entry into the computer, monitors patient profiles for appropriate medication use oversees the work of the support staff and documents any relevant information. Performs clinical duties including medication therapy management and clinical monitoring with a goal of positive clinical patient outcomes. The position is expect to meet state certifications, requires familiarity with standards concepts, practices, and procedures, and relies on experience and judgement to plan and accomplish goals. 

Experience - Retail experience and working knowledge of 340B and Patient Assistance programs.

Education - Bachelor's Degree

License - State Appropriate Pharmacy License  

  1. Pharmaceutical Care
    1. Reviews patient information to evaluate appropriateness of drug therapy and dosing regimens as well as absence of contraindications including but not limited to drug-drug, drug-food, drug-laboratory, and drugdisease interactions and communicates clinically relevant interactions to prescribers and department staff as appropriate. Recommends drug discontinuation or dosage alteration when indicated.
    2. Assist the medical staff and department staff in selecting therapy regimens which are safe, cost effective, and meet individual patient needs . Suggest non-drug therapy when appropriate. Suggest non formulary drug therapy when appropriate.
  2. Liaison/Committee Participation
    1. Serves as a liaison to nursing staff by completing routine rounds with unit leaders, or serves on and/or participates in committee. Committee participation is not limited to the pharmacy department alone.
  3. Education
    1. Completes all department wide and area specific competencies on time
  4. Quality Management
    1. Initiates or participates in at least 1 quality improvement initiative to improve efficiency, reduce waste, improve patient safety, or employee satisfaction using the PDCA model.
    2. Reviews and abides by departmental standard operating procedures around workflow, pharmacy automation, or environmental monitoring standards
