 Job description - Resident Assistant - Overnights 

$17-$20/ hr - based on experience - $1.25/hr for Shift Differential 

Must be 18 years or older to apply The Caregiver is a part of the Cornerstone team, a resident-focused, high performance, high commitment work team that supports the moto of, “creating healthy living and work environments.”  As a Caregiver, they are responsible for ensuring that quality services are a way of life for this community where every associate’s interaction with residents and family members reflects the philosophy of Cornerstone Management Services. A Caregiver is trained to provide personal care and related services to residents in the community according to documented, individualized service and care plans.

Overview: Obtains vital signs, records, and notifies the supervising nurse of deviations per Individual Service Plan (ISP).

Responsibilities:

  • Follows the resident’s ISP as directed.
  • Follows the daily task sheets developed for each resident.
  • Communicates with the appropriate supervisor about changes in the resident’s status and/or the home situation
  • Utilizes standard precautions and infection control procedures.
  • Follows written instructions and requests assistance and further instructions with new or unfamiliar situations.
  • Prepares resident progress notes and submits this documentation in a timely manner.
  • Assists the resident in maintaining a safe and clean environment.
  • Exhibits a caring and respectful attitude toward the resident and the resident’s property.
  • Works with the Resident Services Director and the rest of the operations team to build relationships and strong communication with residents and families
  • Assists with activities and encourage resident engagement and participation
  • Upholds confidentiality of residents and coworkers, HIPAA
  • Attends bi-weekly in-service meetings and mandatory training.
  • Adheres to all community and company policies
  • Understands proper use of facility equipment.
  • Reports observations concerning structural and equipment wear, defects, and malfunctions to the supervisor.
  • Performs any other duties as assigned.

May aid with the following:

  • Bathing, assisting in dressing and undressing residents as needed.
  • Assisting with grooming needs such as hair care, mouth care and nail care.
  • Transporting residents and assisting with walking.
  • Assisting with toileting as needed.
  • Changing incontinent briefs and giving perennial care.
  • Changing bed linens and making beds.
  • Assisting in recording temperature, blood pressure, pulse and respiration rates, and food and fluid intake and output, as directed.
  • Aiding in the delivery of specific core programs, such as falls, prevention, exercise, and others as identified, which will provide entertainment, socialization, exercise, relaxation and fulfillment of spiritual needs to all residents.
  • Positioning.
  • Medication reminders.
  • Shares resident success stories and best practices to demonstrate the impact of improved quality of service provided.
  • Participates in family conferences as needed to discuss the resident’s status and review the appropriate service plan, medication regimen.
  • Reports any concerns or changes in resident’s physical, intellectual or emotional well-being to the RSD or QMAP.
  • Creates and maintains resident records by properly creating resident records on move-in, documenting all medications properly, using proper notation on progress notes, maintaining outside provider laboratory reports, creating, and maintaining all reports including incident and death reports, completing transfer forms, providing quarterly report care summaries
  • Housekeeping and Safety Compliance Duties include cleaning assigned areas, following cleaning schedule, learning and demonstrating proper cleaning solutions, identifying solutions, displaying knowledge of solution apparatus, familiarizing self with Universal Work Precautions.
  • Housekeeping duties may include, but are not limited to, laundering linens and personal clothing, cleaning bathrooms, taking out the trash, spot cleaning carpets, and changing light bulbs.
  • May perform or assist with kitchen duties as necessary and requested by supervisor.
  • Carries out special duties or projects as assigned.
