Job Objective:

Plans, organizes, coordinates, and executes all functions and activities associated with the 24/7 Recognition Program, requiring open, frequent, and responsive communication to meet 24/7 donor needs. Acts as the key point of contact within the healthcare system for program donors, ensuring timely access to a variety of healthcare programs and services in gratitude for their contributions.

Education:Required: Bachelor's degree in Nursing or Master's degree in Nursing required if hired into position after October 1, 2023Licensure/Certification:Required: California Registered Nurse (RN) License; American Heart Association BLSExperience:Required: Five (5) years of recent patient care experiencePreferred: Cumulative patient care experience in a variety of care delivery settings (acute, ambulatory, case management, etc)Reports To: Director-24/7 ServicesSupervises: N/AAges of Patients: All AgesBlood Borne Pathogens: Minimal/ No Potential

Essential Responsibilities

1.Demonstrates compliance with the Code of Conduct and compliance policies, taking action to resolve compliance questions or concerns and report suspected violations.2.Collaborates with 24/7 leadership and peers to streamline workflow processes, ensuring high-caliber service while conserving valuable health system resources.3.Assists with and is accountable for coordinating safe, high-quality patient care; integrates services and prescribed treatments among the hospital system's different programs, service sites, departments, and physicians in a manner that is valued and acceptable to all stakeholders, especially 24/7 members.4.Collaborates with 24/7 members and/or their designees to coordinate and schedule all medically related appointments.5.Facilitates 24/7 members' participation in all decisions concerning their care and advocates appropriately on their behalf.6.Adheres to health system scripting and seeks assistance when unclear.7.Recognizes appropriate and opportune moments to capture and relay vital information to The Foundation to support philanthropic activities and member retention.8.Ensures physician orders to initiate treatments and scheduling are available and viable; judiciously intervenes or connects with physicians to rectify issues.9.Intervenes appropriately to reduce anxiety and enhance patients' ability to follow through with appointments and prescribed care.10.Communicates and bridges knowledge gaps, utilizing teachable moments to provide up-to-date medical information on diagnostic testing and planned treatments; wisely recognizes when a member's need for information or support is beyond personal knowledge and facilitates timely contact with the appropriate entity.11.Escorts 24/7 patients to and from various departments for tests and treatments; assesses the need for adaptive equipment, wheelchairs, etc., to support safety. Remains with patients as requested and proactively orchestrates or takes additional customer service measures as appropriate to ensure a high-quality patient experience.12.Recognizes potentially critical situations and takes action to minimize adverse outcomes.13.Integrates with the Emergency Department (ED) team and acts as an adjunct caregiver to facilitate quality care. Monitors treatment activities, tests, and medication orders while advocating to support timeliness. May assist, in close collaboration and with permission from the primary assigned ED nurse, with the delivery of general nursing care to improve the patient experience (excluding assessments and medications). Takes action to begin work on referrals or prescribed follow-up measures upon discharge.14.Collaborates with in-patient staff to facilitate a smooth hospital admission and stays with the patient throughout the admission process as appropriate. Assures that the patient will follow up on their care daily.15.Conveys through actions and communication an appreciation for the central role 24/7 donors play in the financial health of the organization and actively promotes positive perceptions of the program.16.Shares knowledge and acts as a professional resource to colleagues and other members of the healthcare team. Trains, orients, coaches, and positively supports new hires to ensure confident, seamless integration into the department, boosting new team members' ability to uphold safety, care quality, and outstanding customer service.17.Embraces change and continuously demonstrates a commitment to utilizing evidence-based practice.18.Seeks opportunities to support and uplift department morale.19.Promotes a positive department image when interfacing with health system staff and professionals and either personally takes action to address issues or immediately informs and engages 24/7 leadership.20.Involves appropriate line managers in service recovery as necessary.21.Provides patients with information on how to contact 24/7 staff.22.Assists other team members when necessary.23.Performs other duties as assigned.
