Job description

Affinity is looking for reliable professionals to join our team.

We have a Trach/Vent dependent patient who needs round the clock care in the 19143 zip code of Philadelphia. We are looking for experience LPN and RN tp provide reliable care as they are discharged into the community. The ideal candidate would be able to work 12 hour shifts 2 or 3 days a week. .

    1) Provide skilled nursing care for trach/vent-dependent patients in the home
    2) Record a patient’s medical history accurately
    3) Take and record measurements of blood pressure, temperature, heart rate etc.
    4) Observe patients under treatment to identify progress, side-effects of medications etc.
    5) Monitor patients’ condition including fluid intake and output and compose patient charts
    6) Administer injections, prescribed medications, enemas etc., clean and dress wounds and assist with other basic medical care tasks
    7) Collect samples for lab testing as assigned
    8) Communicate with PCP, and other stakeholders as need to coordinate appropriate care.
    9) Provide emotional and psychological support when needed
    10) Communicate with patients’ family or friends to provide advice, comfort and release instruction
REQUIREMENTS:
    1) Active LPN License in Pennsylvania (RN welcome to apply)
    2) Experience with trach and vent care required
    3) Proven ability to work independently and as part of a multidisciplinary team
    4) Familiarity with health and safety protocols and nursing best practices
    5) Strong communication and interpersonal skills
    6) Physically able to perform all nursing duties
    7) Compassionate, reliable, and able to manage stress in a home care environment
    8) Knowledge of Axxess and HHAeXchange software is a plus
