What You'll Do
  • Complete physical therapy evaluations, reassessments, and discharge visits
  • Develop individualized plans of care based on patient goals
  • Provide skilled physical therapy services in patients' homes
  • Complete Starts of Care and ongoing patient visits
  • Educate patients and caregivers on therapeutic exercises, mobility training, fall prevention, and home safety
  • Collaborate with physicians and the interdisciplinary care team
  • Complete documentation electronically using WellSky on a company-issued tablet

You'll provide home health services throughout the New River Valley, including:

  • Blacksburg
  • Christiansburg
  • Radford
  • Pulaski
  • Floyd

  • Surrounding communities

Patient visits are scheduled geographically to promote efficient travel and maximize your time with patients.

Qualifications
  • Active Virginia Physical Therapist License Required
  • Minimum of One Year of Physical Therapy Experience
  • Home Health Experience Preferred but Not Required
  • OASIS Experience Preferred but Willingness to Learn Required
  • Valid Driver's License and Reliable Transportation
  • Comfortable Working Independently in Patients' Homes
