Job description

Legacy Concierge is looking for high-quality caregivers who continuously demonstrate excellent communication skills, strong interpersonal relationships, and quality care delivery through a person-centered care approach. 

Caregivers provide non-medical companionship care to clients who require supervision due to advanced age, physical disabilities, or cognitive impairments. In addition to offering companionship, caregivers assist clients with activities essential to independent living that they are unable to perform on their own due to health or age-related limitations. Caregivers are responsible for completing appropriate forms and documentation related to client care, maintaining communication with office staff during emergencies, and adhering to rules and regulations regarding client health and medical information.  

It is understood that some clients, due to their mental condition, may exhibit combative behavior. Caregivers are tasked with ensuring the safety of such clients and preventing harm to the clients themselves or others. Thus, by accepting a job as a caregiver, and to the maximum extent allowed by law, caregivers assume the risk that clients may be combative and that they may be injured. If a caregiver is uncomfortable with these responsibilities or the associated risks, they must immediately notify Legacy Concierge management. By not notifying management, caregivers affirm their ability to perform the essential duties of the position, with or without reasonable accommodations, and accept the inherent risks of the role. 

Key Responsibilities 

Caregiver Responsibilities 

  • Deliver 1:1 personalized non-medical care in the client's home. 

  • Support activities of daily living (ADLs) such as but not limited to: personal care, transfer assistance, bathing, dressing, incontinence care, medication reminders, proper hand hygiene, etc. 

  • Support instrumental activities of daily living (IADLs) such as but not limited to: meal preparation, light housekeeping, transportation (if applicable), companion care, etc. 

  • Complete daily care notes accurately and in a timely manner using agency document systems. 

  • Experience providing care and support to clients with a variety of conditions, including but not limited to: Alzheimer's, dementia, Parkinson's, post-operative recovery, MS, short- and long-term memory loss, COPD, CHF, etc. 

  • Communicate promptly and consistently with the RN Care Ambassador and/or Client Care Coordination team when there is a change in client status, incident, or general concerns requiring escalation. 

  • Maintain a reliable schedule, arrive on time, and communicate any scheduling changes according to company policy. 

  • Demonstrate Legacy Concierge's core values and client-centered care principles at all times. 

  • Comply with rules and regulations pertaining to client health information, specifically HIPAA’s policies and procedures. 

  • Promote positive, supportive, and respectful communication to client/family and Legacy Concierge. 

  • Maintain confidentiality of all clients and company information at all times, in full compliance with HIPAA, infection control standards, and internal protocols. 

Required Qualifications 

  • At least 5 years of professional experience or personal caregiving experience and/or equivalent education or clinical hands-on experience 

  • Registered Home Care Aide in good standing (CA only) 

  • Proof of negative tuberculosis test/chest x-ray 

  • Demonstrated ability to work competently in a 1:1 private care environment. 

  • Ability to work independently and follow instructions and ask questions when clarification is needed. 

  • Strong communication, documentation, professionalism, and customer service skills. 

Physical Requirements 

  • Ability to stand, sit, kneel, and move for extended periods of time. 

  • Ability to lift, transfer, or reposition patients with appropriate assistive techniques. 

  • Ability to lift or carry equipment and supplies up to 50 pounds. 

  • Use of technology equipment including tablets, phones, and navigating through WellSky, Learning Management System, etc. 

  • Ability to safely and confidently use durable medical equipment (DME)
