SUMMARY

The Social Worker provides critical support to individuals and families navigating illness and disabilities. This role helps resolve personal, financial, and environmental challenges while working in accordance with ADAPT Community Network (ADAPT) policies, regulatory requirements, and standards of the profession. The Social Worker empowers families to understand and negotiate educational and clinical systems, advocating for their needs. Additionally, this position fosters collaboration with other disciplines to ensure a comprehensive continuum of care and support.


ESSENTIAL DUTIES AND RESPONSIBILITIES

The duties and responsibilities of the Social Worker will include, but are not limited to the following:

โ€ข Participates in the Adapt Intake Processes

โ€ข Develops case notes, case summaries / social histories, etc. for each student on his/her caseload including appropriate recommendations.

โ€ข Assists families in obtaining services such as housing and residential placement, food stamps, Medicaid, Assistive Technology services; respite services; family reimbursement services; camp, vacation referrals, etc.

โ€ข Integrates therapeutic intervention techniques into school programs or home of students served through consultation with parent/guardian and staff at meetings, case conferences or through other informal contacts.

โ€ข Participates in interdisciplinary team meetings and annual reviews for purposes of planning, implementation, and monitoring of IEPโ€™s.

โ€ข Provide emotional and behavioral support to students, particularly those with disabilities.

โ€ข Lead the schoolโ€™s efforts to ensure students receive necessary social-emotional resources or

โ€ข Lead the school's efforts to provide students with the emotional support needed for academic, social, and emotional success.

โ€ข Facilitates communication with family and/or caregivers concerning issues such as attendance, transportation, medical status, and clearance, etc.

โ€ข Conducts parent trainings and monthly workshops for parents in conjunction with other multidisciplinary team members

โ€ข Responding promptly to family concerns and providing guidance on available resources.

โ€ข Documents status of all active cases.

โ€ข Assists with other duties such as arrival and dismissal, feeding, bath rooming and classroom management as needed.

โ€ข Keeps abreast of changes and research in the field of Social Work.

โ€ข Assures confidentiality of all information regarding students and families receiving services.

โ€ข Communicates with staff about goals, rights of persons receiving services, confidentiality, family interaction, etc.

โ€ข Participates in activities pertaining to student transitioning/Turning-Five Process

โ€ข Serves and participates as a member of interdisciplinary team meetings, annual reviews, parent meetings, Special Review Committee meetings, In-Services, etc. as required.

โ€ข Participates in in-service training programs as required.

โ€ข Provides supervision and training for Social Work interns and novice Social Workers.

โ€ข Attends professional meetings and conferences.

โ€ข Performs other related duties as requested.


QUALIFICATIONS

โ€ข Ability to read, write, speak, and understand English (Bilingual a plus)

โ€ข Ability to communicate effectively orally and in writing

โ€ข Good interpersonal skills are required to interact effectively with employees, co-workers, students, families, and outside contacts.

โ€ข Good organizational skills and the ability to meet deadlines

โ€ข Proficiency in the use of a personal computer and knowledge of appropriate software

โ€ข Due to the sensitive nature of the position, one must be able to maintain the strictest confidentiality.

โ€ข Ability to work independently and as a part of the team

โ€ข Ability to multi-task

โ€ข Ability to work flexible hours

โ€ข Demonstrate sound judgment and the capacity to respond to unusual situations.

โ€ข Keen ability to analyze problems and determine corrective actions

โ€ข Must be available Monday-Friday, 8am-3pm


EDUCATION and/or EXPERIENCE

โ€ข Minimal qualifications must have master's degree in social work from an accredited program

โ€ข NYS Licensed Social Worker with at least 1 year of prior experience as a Social Worker preferred


LICENSE/CERTIFICATION

โ€ข New York State Bilingual Spanish Extension

โ€ข New York State Licensed Master Social Worker (LMSW)

NOTE: Individuals with New York State Certification in Social Work (CSW) have certification that was automatically converted to the New York State Licensed Master Social Worker (LMSW).


COMPENSATION: $43.02/hour + $1,500 Sign on Bonus + Industry-Leading Benefits
