Nurse Practitioner (FNP or AGNP) โ€“ Post-Acute Care-1099 (Contractor)
Location: Seguin, TX | Schedule: Days (Flexible)

Compensation:ย 60% of monthly collections

Overview

We are seeking a compassionate and motivatedย Nurse Practitioner (FNP or AGNP)ย to join our post-acute care team. This role focuses on delivering high-quality patient care inย skilled nursing facilitiesย and supporting the recovery and management of medically complex patients. The ideal candidate will demonstrate strong clinical acumen, excellent communication skills, and a collaborative mindset to ensure the best outcomes for our patients.

Responsibilities

  • Perform comprehensive health assessments and physical exams for nursing home patients.
  • Develop and implement individualized treatment and care plans.
  • Diagnose and manage acute and chronic conditions in the post-acute setting.
  • Educate patients and responsible parties on health maintenance, disease prevention, and medications.
  • Accurately document visits and maintain compliance with HIPAA and clinical guidelines.
  • Collaborate with physicians, facility staff, and interdisciplinary teams to coordinate care.
  • Participate in quarterly provider meetings and take weekday call (Mondayโ€“Friday, 7 AMโ€“7 PM), as needed.

Qualifications

  • Board Certified Nurse Practitioner (FNP or AGNP)
  • 1โ€“2 years of clinical experience preferred
  • Skilled Nursing Facility (SNF) RN experience is a plus
  • Must be able to round on patients inย Seguin, TX, twice a week
