Home Visiting Nurse Practitioner (1099 Contractor)

Responsibilities: Manage the transitional and chronic care needs of homebound patients including:

• Assessment and evaluation of current condition• Obtain past medical history
• Review symptoms
• Conduct general physical examinations
• Review and reconcile medications
• Screen for depression
• Screen for alcohol and drug abuse
• Identify diagnoses to support care management and treatment planning
• Recommend appropriate measures, including medication prescriptions and skilled care needs
• Efficiently communicate assessment findings to the office/supervising physician
• Educate patients on disease processes, prevention, medications, and compliance

Qualifications:

• Current Nurse Practitioner license in the state of Georgia with no sanctions or restrictions• Minimum of 1 year of clinical experience
• Clinical background in Adult, Family, or Geriatric specialties preferred
• Experience using Electronic Health Records (EHR)
• Strong communication skills
• Comfortable using smartphone and email
• Reliable transportation required
• Ability to work independently
• Home care or home visiting experience preferred
• Must maintain active individual malpractice insurance coverage
• Willingness to spend 30 minutes to 1.5 hours with patients in their homes, which may include pets and/or household tobacco use
