Job description

Hospice Crisis Care LPN - Making Every Moment Matter

This is a PT night shift position 8pm-8am and will require driving to patients within the Dayton/Springfield area. This position will provide 1:1 with the patient at the bedside for patients that are actively declining.

As a Hospice Crisis Care LPN you will provide care for patients in families in homes/facilities, under the direction and supervision of an RN.

Qualifications for the Hospice Crisis Care LPN:

  • Registered LPN with the State of Ohio without any board actions.
  • One year of nursing experience preferred.
  • CPR Certified.
  • Computer skills sufficient to properly document services and care.
  • Ability to drive during daytime, nighttime or inclement weather.
  • Valid Driver's License with Safe Driving Record.
  • State Minimum Automobile Insurance Coverage.
  • Must be able to pass a criminal background check.
  • Must be able to pass a 10 panel drug screen.
