Practice Setting: Hospital Call 90% Labor and Delivery Reason for Coverage: Continued Growth and Exiting Provider Dates of Coverage: TBD Call Schedule: Two Weekends a Month Friday, Saturday and Sunday per Above Call Ratio: 1:4 Currently Total Number of Beds: L&D/Post Partum/Triage 34 Beds Hospital 205 Average Number of Scheduled Patients Per Day: Varies Number of Physicians in Practice: Four No Other OB/GYNs Provide Coverage During Locums Coverage Dates How Many APPs Provide Coverage: 1 per 12 Hour Shift Onsite Lab & Ultrasound: Yes Describe Support Staff: L&D Nurses and Midwives What Percentage of Practice is Low Risk Pregnancy: 60% What Percentage of Practice is High Risk Pregnancy: 40% What Percentage of Practice is GYN: 60% Average Number of GYN Surgeries Per Day: Five What Percentage of Patients are C-Section: One Average Number of Nursery Admissions Per Day: Four Nursery Level (I, II, III): Level II Documentation System/EMR: EPIC Required Obstetrics Procedures: Low Risk Pregnancy (i.e. History of Twins, Toxemia, Pyelonephritis, etc.), High Risk Pregnancy (i.e. with Toxemia, Diabetes, Class II or Higher Cardiacs, Chronic Lung Problems, Chronic Renal Disease, etc.), FHR Interpretation – NST/CST/FSE, OB Ultrasounds – Performance of Interpretation, OB Ultrasounds – Performance of Level I/Screening (Viability) Labor and Delivery Required Procedures: Routine Delivery – Vaginal, Low Forceps, Decision-Making for C-Section, Complicated Delivery – C-Section, Breech, Multiple Births, Abruptions, etc, VBAC's, Repair of 3rd & 4th Degree Lacerations Gynecology Required Procedures: Surgical Gynecology – Open, Laparoscopic Diagnostic/Minor Only-Tubal Ligation, LOA's, Chromal Tubation, Operative/Major (Includes Diagnostic) – Ectopic Pregnancy, Ovary Removal, Cystectomy, Extensive LOA's Estimated Credentialing Timeframe: 30-90 Days
