Job description

Home Personal Care (HPC) Direct Support Professional

Job Summary

We are seeking compassionate, dependable, and motivated Home Personal Care (HPC) professionals to provide person-centered support to individuals with developmental disabilities in their homes and the community. The ideal candidate will promote independence, dignity, safety, and community inclusion while assisting individuals with their daily living needs.

Essential Job Responsibilities

Provide respectful, person-centered care in accordance with each individual's Individual Service Plan (ISP).

Assist with activities of daily living (ADLs), including personal hygiene, grooming, dressing, and mobility as needed.

Prepare meals and assist with meal planning while following dietary guidelines.

Administer or assist with medications as authorized and document appropriately.

Perform light housekeeping duties to maintain a clean, safe, and organized home environment.

Assist with grocery shopping, errands, and other daily household needs.

Provide safe transportation to medical appointments, recreational activities, community events, shopping, and other scheduled outings.

Encourage community participation, social engagement, and skill development.

Monitor the individual's health, safety, and overall well-being, reporting concerns promptly.

Maintain accurate documentation of services provided, medication administration, incidents, and daily progress notes.

Follow all agency policies, DODD regulations, HIPAA requirements, and safety procedures.

Communicate effectively with supervisors, family members, case managers, and other support team members.

Promote the individual's rights, independence, privacy, and dignity at all times.
