Cap City Supported Living & Home Care Services

Cap City Supported Living provides support services geared toward young adults and adults who live with developmental disabilities. Our clients benefit from socialization and community- bases programs, and Home Personal Care.

Essential Job Functions:
Individuals/Staff:
•    Ensure health and safety of the individuals and staff by continuously monitoring and reporting any changes in individuals medical, health and safety needs. 
•    Attend all required agency, medical and individual service-related meetings and appointments.  
•    Serve as a role model and Cap City representatives for individual and staff in tone, morale and professionalism and interactions with individuals, employees, individual’s family, friends, guardians, and any circle of supports for the individuals. 
•    Demonstrate correct procedures in personal and agency goals through the individual ISP, BSP, and other goals such as outcomes, diet, etc.
•    Ensure follow up on all quality assurance and medical reviews as required.
•    Prepare documentation for service for each assigned individual, monitors and documents accuracy.
•    Document and review unusual and major unusual incidents prior to submission.
•    Facilitates appropriate meal planning related to specific individual needs.
•    Oversee the initial and ongoing training of new staff and ensure that staff complete required training and documentation within the time frame specified by Administration.
•    Give staff positive and constructive feedback on job performance in a prompt, tactful, direct and consistent way and report issues to your supervisor. 
•    Adhere to and relay Cap City policy and procedures, licensing requirements, and other company information in a positive, supportive and professional manner. 
•    The scheduling of staff aligning with the 20/20 and ensure accurate reporting of billed hours through agency time and attendance.
•    Ensure proper shift coverage for the site, individual and staffing emergencies.  Working opens shift as well as monitoring the overtime budget. 
•    Ensure staff attend trainings and meetings to adhere to employee requirements
•    Comply with DSP job duties/description
•    Any other duties as assigned. 
Household:
•    Ensure that the individuals clothing and belongings are clean, up to date, and in good condition and that the property is cared for and maintained. 
•    Facilitate shopping as needed.
•    Ensure individuals home is free of damage or health and safety concerns by doing monthly checks.
•    Oversee purchases of individual personal and household supplies to ensure appropriate level of supplies are maintained. 
•    Ensure household meets all private regulatory requirements (i.e. creative housing, landlord, leasing)
•    Oversee the preparation and implementation of emergency evacuation and safety procedures.
•    Plan and manage the household budget.
•    Establish and manage schedules for activities inside and outside of the home.
•    Complies with OSHA, HIPAA, Rights of Individuals (5123.62 ORC), Individuals Options Waiver/HPC (5123:2-13-04-OAC)
•    Advocating to protect and advance the rights of individuals.
•    Embrace and embody the mission, vision and policies and procedures of Cap City.
Essential Qualifications:
•    Minimum of one year of experience in the DD industry or related field. 
•    High School Diploma or equivalent.
•    Valid Driver’s License (no more than 6 points)
•    Current Proof of auto insurance (state minimum)
•    18 years or older
•    FBI and or BCI Background Check (5123:2-1-05.1 ORC)
•    CPR and First Aid Training (5123:2-2-.1 ORC)
•    8hr DODD Training
•    Medication Administration Training
•    Any other trainings deemed necessary by Cap City Home Care Services
Working Conditions:
•    Work is performed primarily in residential setting however traveling within city limits is a frequent requirement. 
•    On Call during designated time frames.  Must be able to respond within 30 minutes of all inquiries and emergencies via in person and 15 minutes by telephone.

Physical Demands:
•    Requires mobility and physical activity: Having adequate range of motion and mobility to work in an office, residentials, or outdoor environment including standing and walking (even and uneven surfaces), sitting for extended period of time, bending, twisting, reaching, balancing, and occasionally lifting and carrying up to 30 pounds. 
•    Requires the ability to defend oneself and individuals in physically aggressive situations through the use of approved descalation techniques.
•    Requires communication, expressing or exchanging ideas by means of spoken and written word.  Talking is required to share information regarding individuals.
•    Requires hearing:  Hearing is required to receive and communicate detailed information thought oral communication and ensure ability to supervise individuals.
•    Requires seeing:  Clarity of vision at 20 inches or less and at a distance to complete paperwork, observe and supervise individual. 
Mental Demands:
•    Requires the ability to reason and be empathetic to individuals and teammates.
•    Requires the ability to cooperate with individuals, teammates, supervisors and outside supports for individuals to exchange ideas, information and instruction. 
•    Requires the ability to work under stress and emotionally changed situations.
