The Social Worker will provide mental health counseling, case management, discharge planning, and reentry coordination for individuals housed. Clinical & Counseling Services
- Provide individual mental health counseling to incarcerated individuals.
- Conduct psychosocial assessments and ongoing evaluations.
- Develop, implement, and monitor individualized treatment and service plans.
- Manage mental health cases from intake through release.
- Coordinate discharge planning and continuity of care.
- Assist individuals with reentry planning, including linkage to housing, healthcare, and social services.
- Collaborate with medical staff, mental health clinicians, and correctional teams.
- Liaise with community-based treatment providers, social service agencies, and reentry programs.
- Ensure smooth transitions between correctional and community-based care systems.
- Maintain accurate and timely clinical documentation.
- Ensure services comply with applicable regulations, policies, and professional standards.
- Participate in interdisciplinary meetings and case conferences as required.
Education: Bachelor's Degree or Master's Degree in Social Work (Required)
Licensure
