Position Overview

Positive Progression Psychology is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in Reading, Pennsylvania. Work Environment

This position allows you to work remotely while serving clients at a hospice center. Our team comprises skilled professionals passionate about mental health and community service. The remote nature of this role offers flexibility, although you should be prepared for occasional evening or weekend commitments during special programs or events.

Key Responsibilities

• Conduct thorough assessments to evaluate patients' emotional, psychological, and social needs, ensuring an individualized approach to care.
• Provide both individual and group counseling, utilizing evidence-based practices tailored to hospice settings, including grief counseling and end-of-life planning.
• Develop personalized service plans that align with clients' identified goals, monitoring progress and making necessary adjustments to enhance outcomes.
• Advocate for clients to help them access necessary resources and support services, ensuring their rights are recognized and met within the healthcare system.
• Collaborate with a multidisciplinary team, including doctors, nurses, and volunteers, to enhance the quality of services provided to patients and their families.
• Engage in program development and evaluation, contribute to grant writing and reporting, and implement evidence-based strategies to improve organizational impact.
• Facilitate community outreach initiatives to raise awareness about mental health, hospice care resources, and available support services.

Patient Population / Client Focus

This role focuses on supporting individuals and families undergoing end-of-life transitions in a hospice environment. Your expertise will be crucial in addressing the unique emotional and psychological challenges faced by clients and their loved ones, offering guidance and support during a challenging time. Clients may include elderly individuals, terminally ill patients, and those experiencing significant grief and loss.

Qualifications

• A Master's Degree in Social Work from an accredited program is essential.
• An active state licensure as a Licensed Clinical Social Worker (LCSW) is required.
• Knowledge and understanding of trauma-informed care practices and social justice principles are critical.
• Strong communication skills, empathy, and the ability to build rapport with clients from diverse backgrounds.

Preferred Experience

• One year of experience in a nonprofit organization or similar mental health setting is preferred, especially in hospice or palliative care environments.
• Experience in advocacy and case management, particularly relevant to healthcare settings.

Compensation and Benefits
