    Location: New York, NY
    Contract Duration: 8 Weeks
    Shift Details:
    Day Shift - 5x8 Hours
    09:00 AM - 05:00 PM
    Monday to Friday


    Job Summary

    "Navitas Healthcare, LLC" is seeking an experienced Case Worker to provide comprehensive social services and case management support to individuals and families in need. The role involves developing and implementing service plans, assessing eligibility for assistance programs, and delivering counseling and supportive services in coordination with community resources.


    Key Responsibilities
    • Provide social services support to clients applying for or receiving public assistance, food stamps, and medical assistance
    • Develop, implement, and monitor individualized service plans
    • Assess client eligibility for various assistance and support programs
    • Conduct counseling sessions and provide ongoing client support
    • Perform investigatory work related to social service cases
    • Collaborate with interdisciplinary teams and community agencies
    • Maintain accurate, timely, and detailed case documentation
    • Support adults requiring protective or institutional care services

    Requirements
    • Bachelor's degree from an accredited college or university required
    • Minimum of 3 years of experience as a Case Worker or in a related social services role
    • Strong English communication skills (speaking, reading, and writing)
    • Ability to manage multiple cases in a fast-paced environment
    • Excellent organizational and documentation skills
