Title: Pharmacist - Outpatient Infusion

Location: Hood River

State: Oregon

Duration: 13 Weeks (Temp to Permanent)

Schedule: Day Shift | 4x10-Hour Shifts
Shift Timing: 7:00 AM - 5:00 PM
Weekly Hours: 40 Hours

Job Summary:

"Navitas Healthcare, LLC" is seeking an experienced Pharmacist to provide pharmaceutical care and medication management services in an outpatient infusion setting. The Pharmacist will evaluate medication orders, prepare and verify medications, collaborate with healthcare providers, and ensure safe and effective medication therapy for outpatient infusion patients. The ideal candidate will possess strong clinical knowledge, infusion pharmacy experience, and the ability to work efficiently in a fast-paced outpatient environment.

Key Responsibilities:

  • Review, verify, and dispense medications accurately and safely

  • Prepare and manage outpatient infusion medications and therapies

  • Monitor medication therapy for safety, effectiveness, and compliance

  • Collaborate with physicians, nurses, and healthcare teams regarding medication management

  • Provide patient counseling and education on medications and infusion therapies

  • Maintain accurate pharmacy documentation and patient records

  • Ensure compliance with hospital policies, pharmacy regulations, and safety standards

  • Monitor for drug interactions, contraindications, and adverse reactions

  • Support quality improvement and patient safety initiatives

Required Skills & Experience:

  • Minimum 1 year outpatient infusion pharmacy experience required

  • Strong knowledge of infusion medications and pharmacy operations

  • Ability to work independently in a fast-paced outpatient setting

  • Strong communication, critical thinking, and organizational skills

  • Experience with Epic charting system preferred

Education Requirements:

  • Doctor of Pharmacy (PharmD) or equivalent Pharmacy degree required

  • Graduate from an accredited pharmacy program required

Certifications & Licensure:

  • Active Oregon Pharmacist License required

  • BLS preferred
