Locations: Home health visits available in Lee county (Fort Myers area)

What You'll Do

  • Provides treatment to the patient in compliance with agency policies and procedures and federal and state regulations.
  • Performs initial and ongoing assessments to determine level of functioning and patient's ability to perform activities of daily living, including OASIS assessments at appropriate time points.
  • Develops and revises a plan of care in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of self-care.

What You'll Need

  • Bachelor Degree in Occupational Therapy.
  • Florida licensure in occupational therapy.
  • Minimum of one (1) year experience as a licensed Occupational Therapist.
  • Home health care experience preferred.
