POSITION SUMMARY
The Community Wellness Department (CWD) is a multi-site program operating in Sanย Francisco and Oakland, that provides culturally sensitive prevention, early intervention, andย supportive social service programming. CWD community programs include social serviceย programming for youth and adults, cultural programming, health education, systemย navigation/ coordination services, and HIV/AIDS prevention and care. Services are rooted inย a holistic provision model that integrates prevention, community engagement, cultural competence and provides linkages to other treatment services.
The Community Wellness Department is seeking a Senior Program Manager to collaborate with the CWD Director to manage day-to-day operations of the department areas thatย include monitoring grant performance, assist with budget management, lead programย planning, guide QMIP and other Quality Improvement initiatives, operationalize reportingย and evaluation. The incumbent will lead, maintain, and expand CWDโs Comprehensiveย
Perinatal Services Program (CPSP).
The Community Wellness Department provides multi-site services in 2 Bay Area Countiesย at 2 locations (1 in SF and 1 in Oakland) to urban American Indians, Alaska Natives, otherย Indigenous people, and diverse members of the surrounding communities.
The Native American Health Center is an accredited institution and adheres to the standards ofย excellence set forth by the Accreditation Association of Ambulatory Health Care (AAAHC) andย the Commission of Dental Accreditation (CODA).
DUTIES AND RESPONSIBILITIES
1. Co-Manage CWDโs day-to-day operations, project performance management, hiring,ย human resources management and ensuring effective spending.
2. Function as an integral member of the departmentโs leadership team, working closelyย with the Director of Community Wellness Department, Senior Program Managers,ย Director of Behavioral Health, and Director of Research & Evaluation.
3. Support departmental strategic planning initiatives and engage in departmentย development, performance management, and team building.
4. Implement and sustain the CPSP Perinatal expansion project and develop workflowsย for cross-functional division, CWD WIC, and Medical
5. Develop and sustain the women's Children and Family service division for CWD, a newย revenue producing multi-departmental service line
6. Develop, implement, and maintain operation processes, procedures, and collaborationย with the State, Federal, and County Home Visiting Programs.
7. Consult with leadership on the program modifications and recommendations for State,ย Federal, and County contracts.
8. Convene and facilitate joint planning to design and implement State, Federal, andย County Home Visiting Programs.
9. Manage and Lead the daily functions of four direct service programs, Tribal Homeย Visiting, Fatherhood, Alameda County Case Management, and CPSP Perinatalย expansion project.
10. Monitor project implementation and program rollout tracking, communications, andย reporting for internal and external stakeholders.
11. Conducts audits of database and relevant processes in order to identify problems andย perform necessary corrections.ย
12. Manage and Lead the daily functions of four direct service programs, Tribal Homeย Visiting, Fatherhood, Alameda County Case Management, and CPSP Perinatalย expansion project.
13. Resolve staffing issues and facilitate conflict resolution for staff under directย supervision. Hire, discipline, evaluate and terminate staff in coordination with theย Human Resources department.
14. Support the execution and implementation of initiatives across the department andย multiples sites, coordinating with site managers, and analyze data to ensureย compliance with accrediting bodies and funding sources.
15. Ensures patient and program data is accurately and completely recorded in theย agencyโs electronic record health system according to contractual timelines andย requirements.
16. Provide leadership on program development and expansion of program activities toย support grant writing. Duties include writing and collecting supporting documents andย directing grant communications with funders.
17. Coordinate activities across departments, sites and with collaborating agencies toย ensure that program areas are meeting goals and objectives.
18. Document processes, procedures, workflows, and meeting agendas/minutes. Conductย presentations on CWD and NAHC programs at the local, state, and national level andย ensure dissemination of information to NAHC staff and community, as appropriate.
19. Member Care: Demonstrate understanding and apply working knowledge of safetyย policies and ensuring safe member practices.
20. Quality Improvement: Actively participate in internal quality improvement teams andย work with members proactively to drive quality improvement initiatives in accordanceย with the mission and strategic goals of the organization, federal and state laws andย regulations, and accreditation standards, when assigned.
21. Creates maintains and distributes reports as needed by program and agency,ย leadership, and diverse funding sources.
22. Foster a team environment to effectively lead and retain your staff. Provide opportunities for staff toย
grow and develop new skills.
23. Must maintain compliance with ergonomic safety standards; be mindful of posture and regularlyย practice ergonomic stretches.
24. Safety: Responsible for ensuring that all duties, responsibilities and operations are performed with theย utmost regard for the safety and health of all personnel involved, including themselves.
25. Safety: Take appropriate corrective actions to address matters pertaining to employee health andsafety that have been brought to their attention.
26. HIPAA: Keep all protected health information (PHI) confidential and abide by HIPAA guidelines for theย release and disclosure of any PHI. Will report unauthorized use of disclosure of PHI immediately, toย Site Director and HIPAA security officer.
27. HIPAA: Ensure the protection of all PHI and compliance of HIPAA guidelines amongst staff underย your supervision. Take appropriate steps to address HIPAA concerns with staff as necessary.

MINIMUM QUALIFICATIONS
1. Masterโs degree in social work, public health, public policy, business or related field,or equivalent work healthcare experience will be considered
2. Five years of experience in progressively responsible positions in non-profit agency.
3. Five years of experience with staff supervision, staff development, and programย development.
4. Experience with program development, implementation and operations.
5. Must be able to provide TB, Physical and Vaccination clearance prior to startย date.
6. CPR certification must be obtained within 90 days and kept current at allย times.
7. Proven fiscal leadership and development skills.
8. Proven leadership and team building skills.
9. Excellent written, verbal and interpersonal skills.
10. Ability to work collaboratively with a broad range of constituents.
11. Experience and ability working with underserved and/or multicultural populations.
12. Knowledgeable about substance abuse and mental health among American Indian/Alaska Native/other Indigenous populations.
