This comprehensive approach to treatment, the gold standard in care for opioid use disorder, has been shown to prevent more deaths from overdose and lead to long-term recovery. Day in the Life of a Care Manager II
  • Provide remote operational support to Care Managers across centers by backfilling vacancies as needed, ensuring continuity of care, maintaining service levels, and minimizing disruptions to daily operations during staffing gaps.

  • Conduct proactive outreach to patients to complete missed care plans, improve care plan compliance, re-engage patients in their care, and support continuity of care through timely follow-up.

  • Identifies patient care requirements by establishing personal rapport with patients and other persons in a position to understand care requirements.

  • Leverage patient data from dashboards and reports to identify trends, monitor care outcomes, prioritize outreach efforts, and take timely action to improve patient engagement, care coordination, and overall clinical performance.

  • Accurately document patient care services by maintaining timely, thorough, and compliant charting in patient records to ensure continuity of care, support clinical decision-making, and meet organizational and regulatory documentation standards.

  • Address patient needs and challenges related to opioid use disorder (OUD) by collaborating with a multidisciplinary care team to develop coordinated care plans, remove barriers to treatment, and support positive patient outcomes.

  • Establish individualized patient care goals in collaboration with patients and the multidisciplinary care team, promoting patient engagement, measurable outcomes, and progress toward improved health and recovery.

  • Assures quality of care by adhering to Crossroads standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments.

  • Schedule and reschedule patient appointments to promote continuity of care, minimize missed visits, improve patient engagement, and ensure patients remain actively connected to treatment and support services.

  • Monitor patients' clinical status by tracking medication adherence, referral completion, and follow-up care, identifying potential barriers to treatment, and coordinating timely interventions to support positive health outcomes and continuity of care.

  • Assist physicians and clinicians with patient onboarding and orientation by ensuring patients understand the care process, completing required intake activities, coordinating initial services, and facilitating a smooth transition into treatment.

  • Maintains patient information confidentiality and privacy.

Education and Experience requirements:
  • At minimum, High School Diploma or GEDrequired.

  • Experience in a fast-paced environment, servicing many clients, is preferred.

  • Experience in healthcare, addiction medicine, community health, care coordinationpreferred.

  • Experience with building individualized care plansis a plus.

  • Knowledge of compliant documentation practices

  • Lived Addiction experienceis a plus.

  • Can-do attitude and willingness to think beyond the status quo.

  • Mission-driven with a passion for helping patients live better, stronger lives.

  • Strong organizational and time management skills with the ability to prioritize multiple tasks in a fast-paced environment.

  • Strong critical thinking and problem-solving skills with the ability to identify barriers to care and implement appropriate interventions.

Hours, Schedule, and Travel
  • Remote role requires living and operating in EST.

  • Expected hours for this role are 37-40 hours per weekwith few opportunities for overtime.

  • Clinic hours range anywhere from 6:30 AM - 9:00 PM. Some weekend shifts may be required
