Key Responsibilities

• Conduct comprehensive assessments of clients' psychosocial needs, taking into account their emotional, mental, and social circumstances.
• Collaborate closely with clients and their families to devise tailored care plans that reflect their individual goals and preferences.
• Provide therapeutic counseling and emotional support to clients, assisting them as they work through transitions associated with detoxification and recovery.
• Advocate for clients’ rights, ensuring they have access to the appropriate community resources and services they need for continued support.
• Engage with an interdisciplinary team—comprised of nurses, caregivers, and other healthcare professionals—to coordinate care and enhance the client’s overall wellbeing.
• Regularly monitor clients' situations through virtual check-ins, addressing changes in their conditions or environments that may require intervention.
• Facilitate discussions and conflict resolution among family members to promote harmonious relationships and effective communication regarding the client's care.
• Inform clients and their families about community resources, support groups, and wellness programs to enhance their social engagement and quality of life.
• Maintain accurate documentation of client interactions, assessments, and progress in accordance with ethical and legal standards.
• Stay updated on best practices in social work, particularly relating to mental health and addiction recovery, to ensure compliance and enhance service delivery.

Patient Population / Client Focus

Your primary focus will be on clients within our detox center, many of whom may be experiencing complex emotional and mental health challenges resulting from substance use disorders. You will work with a diverse client population, including seniors, and will help them navigate various social determinants that affect their mental health. Your empathetic approach will be crucial in fostering trust and encouraging clients to actively engage in their recovery journeys.

Qualifications

• Master's degree in Social Work from an accredited institution is required.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience in a clinical social work role, particularly within a community center or geriatric service setting.
• A solid understanding of social work principles, mental health issues, and available community resources.
• Strong assessment, counseling, and critical-thinking skills focused on the unique needs of clients.
• Demonstrated ability to empathize and build rapport with clients and their families, providing a safe space for healing.
• Excellent communication and interpersonal abilities, facilitating effective collaboration with interdisciplinary teams.
• Proficiency in electronic health record (EHR) systems and various computer applications necessary for documentation.

Preferred Experience

• Experience specifically in detox or substance abuse recovery settings is advantageous.
• Knowledge of geriatric social work practices is preferred, given our focus on supporting senior clients.
• Familiarity with community resources available for mental health and addiction recovery would be beneficial.

Compensation and Benefits
