Key Responsibilities

As a Home Care Physical Therapist, your primary responsibilities will include:

• Conducting thorough evaluations of patients recovering from injuries, surgeries, or illnesses in their home environments.
• Developing and implementing individualized treatment plans based on patient assessments, utilizing techniques including exercise, hands-on therapy, and assistive devices to address pain and mobility challenges.
• Tracking and documenting patient progress and modifying treatment plans as needed to ensure optimal recovery outcomes.
• Educating patients, caregivers, and families on effective therapeutic exercises, assistive devices, and ongoing management of physical therapy procedures.
• Maintaining updated knowledge of industry best practices through attendance at workshops, professional literature, and networking within the field.

Patient Population / Client Focus

Our services cater to a diverse patient population ranging from children to older adults. You will work with individuals facing a variety of diagnoses, including but not limited to orthopedic injuries, neurological disorders, post-surgical rehabilitation, and chronic pain conditions. Your role is pivotal in inspiring hope and motivating patients to achieve their rehabilitation goals within the comfort of their homes.

Qualifications

To excel in this role, you must possess:

• A Doctorate in Physical Therapy (DPT).
• A valid state license to practice physical therapy.
• Strong interpersonal and communication skills, both written and verbal.
• Basic proficiency in word processing and computer skills.
• Flexibility and the capacity to adapt to varying patient needs and environments.

Preferred Experience

While prior work experience is not an essential requirement, candidates with two years of experience in a related field will be viewed favorably. Experience in home health settings or with a diverse patient demographic will enhance your integration into our team.

Compensation and Benefits
