Nurse Practitioner (NP) – In-Home Risk Adjustment Assessments
Location: Lane County, OR

We’re hiring experienced Nurse Practitioners to join a growing team that specializes in in-home risk adjustment and wellness assessments for Medicare and other vulnerable populations.

This is an opportunity to make a measurable impact in value-based care, helping ensure accurate documentation, care gap closure, and individualized health education—all from the comfort of your patients’ homes.

What You’ll Do

  • Conduct in-home annual wellness visits and risk adjustment assessments

  • Evaluate patients with chronic conditions through:

    • Comprehensive physical exams

    • Medication reviews

    • Cognitive, mental health, and fall risk screenings

    • Social and functional assessments

  • Educate patients on health maintenance topics such as medication adherence, pain management, and chronic disease self-care

  • Help close quality care gaps (labs, screenings, immunizations)

  • Accurately document assessments in a user-friendly EMR

Qualifications

  • Active, unrestricted Nurse Practitioner license in OR

  • 3+ years of experience in patient care (geriatrics, primary care, or home health preferred)

  • Previous experience in risk adjustment or in-home assessments preferred

  • Comfortable working independently in a field-based environment

  • Must have reliable transportation and a willingness to travel
