Nurse Practitioner (NP) – In-Home Risk Adjustment Assessments
Location: Lane County, OR
Do you thrive in a patient-centered environment, value autonomy, and want to help improve health outcomes for at-risk populations? We’re hiring experienced Nurse Practitioners to join a growing team that specializes in in-home risk adjustment and wellness assessments for Medicare and other vulnerable populations.
This is an opportunity to make a measurable impact in value-based care, helping ensure accurate documentation, care gap closure, and individualized health education—all from the comfort of your patients’ homes.
What You’ll Do

Conduct in-home annual wellness visits and risk adjustment assessments

Evaluate patients with chronic conditions through:

Comprehensive physical exams
-
Medication reviews
-
Cognitive, mental health, and fall risk screenings
-
Social and functional assessments
-
-
Educate patients on health maintenance topics such as medication adherence, pain management, and chronic disease self-care
-
Help close quality care gaps (labs, screenings, immunizations)
-
Accurately document assessments in a user-friendly EMR
Qualifications
-
Active, unrestricted Nurse Practitioner license in OR
-
3+ years of experience in patient care (geriatrics, primary care, or home health preferred)
-
Previous experience in risk adjustment or in-home assessments preferred
-
Comfortable working independently in a field-based environment
-
Must have reliable transportation and a willingness to travel
