Position Overview

Thrive Compassionate Counseling is seeking a dedicated and skilled Licensed Clinical Social Worker (LCSW) to join our dynamic rehabilitation team. This full-time, remote role will play a crucial part in supporting our clients' emotional and social needs as they navigate the journey to recovery and reintegration into their communities. You will work closely with a diverse patient population, conducting assessments, providing counseling, and collaborating on individualized treatment plans.

Work Environment

This role is fully remote, allowing you the flexibility to work from the comfort of your own home. Key Responsibilities
  • Conduct comprehensive psychosocial assessments through interviews, observations, and reviewing medical records to understand each patient's unique emotional, social, and environmental challenges.
  • Provide therapeutic counseling and emotional support to patients and their families, addressing adjustment issues, grief, and coping strategies that arise during the rehabilitation process.
  • Collaborate effectively with the rehabilitation team, contributing insights into each patient’s psychosocial needs and assisting in the development of goal-oriented treatment plans.
  • Assist patients in navigating community resources, such as connecting them with support groups, vocational rehabilitation services, or housing assistance, to ensure a successful transition post-rehabilitation.
  • Educate patients and their families about available resources and post-rehabilitation services to enhance their understanding of the continuum of care.
  • Participate in discharge planning, facilitating smooth transitions for patients returning home or moving to other care settings.
Patient Population / Client Focus

Our patients come from various backgrounds and may be dealing with conditions such as stroke, spinal cord injuries, orthopedic surgeries, and neurological disorders. As an LCSW at Thrive Compassionate Counseling, you will advocate for this diverse population, playing a pivotal role in addressing their mental health and social needs as they pursue rehabilitation. Your contributions will help empower patients towards achieving maximum independence and quality of life.

Qualifications
  • A Master's degree in Social Work from an accredited institution.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW).
  • Strong understanding of psychosocial aspects of rehabilitation, including adjustment issues, grief, and coping mechanisms.
  • Exceptional communication and interpersonal skills are essential for effectively interacting with patients, families, and the rehabilitation team.
Preferred Experience
  • Previous experience working in a rehabilitation or medical setting is highly preferred, as it offers a better understanding of the unique challenges faced by patients in recovery.
  • Experience in providing counseling services to individuals transitioning through significant life changes can significantly enhance your effectiveness in this role.
Compensation and Benefits
