Location: Smethport, PA
Schedule: 1-5 days per week
Employment Type: W-2
Compensation: *Offering up to $15,000 sign-on bonus*
wRVU pay model; assuming full-time, 75 visits/week (15 visits/day), a PMHNP could estimate they will earn $140k - $160k/year.

We are seeking experienced Psychiatric Mental Health Nurse Practitioners (PMHNPs) to serve as in-house SNF-ists, providing psychiatric and behavioral health care to geriatric residents in Skilled Nursing and Assisted Living settings. This role is ideal for a clinically confident provider who values autonomy, collaboration, and making a meaningful impact in long-term care.

What you'll do
â—† Serve as the facility's leading provider of internal medicine services and day-to-day clinical leader.
◆ Oversee day-to-day care for the entire resident population — short- and long-term — with facility nursing staff support.
â—† Provide comprehensive health assessments and differential diagnoses, managing acute and chronic illness under protocol.
â—† Perform comprehensive physical examinations of patients.
â—† Prescribe all necessary medications and treatments.
â—† Conduct monthly stability visits for long-term residents.
â—† Evaluate residents with changes in condition and participate in Advanced Care Planning.
â—† Develop and implement patient management policies and procedures.
â—† Collaborate and communicate with interdisciplinary teams, medical directors, attending physicians, and specialists.
â—† Complete accurate documentation and CPT coding.
â—† Facilitate referrals to other healthcare professionals and medical facilities.
What we're looking for
â—† Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
â—† Active license and DEA registration.
â—† Master's Degree (required).
â—† Excellent written and verbal communication skills, with the ability to build rapport with patients and their loved ones.
â—† Strong leadership experience with emphasis on teamwork in a healthcare setting.
â—† Ability to thrive in a fast-paced environment.
â—† Preferred: minimum one year of experience in a Skilled Nursing setting.
Compensation amp; benefits
W-2 employment on a flexible, provider-chosen schedule — two nonconsecutive weekdays, daytime hours. Comprehensive benefits package (full-time benefits available at 30+ hours/week) including medical and prescription coverage (Blue Cross Blue Shield), dental and vision (UnitedHealthcare), a 401(k) through Nationwide, voluntary Aflac benefits, 15 days Paid-Time Off, six paid holidays, occurrence-based malpractice coverage ($1M/$3M), a $1,000 annual CEU, DEA, and licensure allowance, mileage reimbursement, and company-provided equipment, including a Microsoft Surface Pro.
