Key Responsibilities

As a Licensed Clinical Social Worker at Synergetic Counseling, your daily responsibilities will include:

  • Conducting thorough clinical assessments of patients with varying mental health issues, creating personalized treatment plans that incorporate evidence-based practices.
  • Utilizing therapeutic modalities such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) to address individual patient needs, adjusting interventions based on ongoing assessments.
  • Maintaining comprehensive and timely clinical documentation, ensuring compliance with HIPAA regulations and other relevant standards to protect patient confidentiality.
  • Collaborating with an interdisciplinary healthcare team to ensure holistic and effective care delivery for each patient.
  • Participating in case consultations and peer reviews, contributing to team discussions aimed at enhancing therapeutic strategies and patient outcomes.
  • Providing essential education and support to both patients and their families regarding mental health challenges and available resources.
  • Staying updated with the latest research and developments in mental health and social work, integrating this knowledge into your clinical practices.
Patient Population / Client Focus

Your work will primarily involve supporting patients in hospice care, many of whom deal with complex emotional and psychological challenges due to chronic or terminal illnesses. You will adopt a sensitive and patient-centered approach to understand their needs, navigating through grief, anxiety, and other mental health concerns with compassion and professionalism.

Qualifications
  • A Master’s degree in Social Work from an accredited institution is required.
  • A valid state license to practice as a Licensed Clinical Social Worker (LCSW).
  • A minimum of 3 years of direct clinical experience providing psychotherapy services in a healthcare setting.
  • Proficiency in solution-based psychotherapy techniques complemented by exceptional communication and interpersonal skills.
  • Strong critical thinking and problem-solving capabilities essential for effective patient interventions.
  • A collaborative mindset, with the ability to work seamlessly as part of a multidisciplinary team.
Preferred Experience
  • Previous experience in hospice or palliative care settings is highly preferred.
  • Familiarity with grief counseling and understanding of end-of-life issues.
  • Proven track record of developing and implementing effective treatment plans that fulfill regulatory and ethical standards.
