Work Environment

The ideal candidate will work onsite within our home care setting, offering the flexibility to visit patients in their residential environments. Our operating hours are Monday through Friday from 8:00 AM to 5:00 PM, which allows for a balanced work-life schedule. As part of our team, you will collaborate with other healthcare professionals to create comprehensive treatment plans that lead to effective, patient-centered care.

Key Responsibilities
  • Conduct thorough examinations and evaluations of patients, identifying functional limitations and disabilities to develop personalized treatment plans.
  • Implement evidence-based treatment protocols while closely monitoring patient progress and making necessary adjustments to the care plan based on real-time data and patient feedback.
  • Document all patient interactions, including initial evaluations, daily encounter notes, interim progress summaries, and discharge planning, ensuring compliance with local, state, and federal regulations.
  • Work collaboratively with other healthcare providers to consult and discuss patient needs, modifications to treatment, and referrals for additional medical or rehabilitation support.
  • Engage in team initiatives aimed at removing barriers to patient access and participating in quality improvement projects that enhance service delivery.
  • Maintain a strong focus on patient education to empower clients in managing their conditions independently.
  • Attend regular team meetings and training sessions to stay updated with best practices and corporate requirements.
Patient Population / Client Focus

This role will focus on a diverse patient population, primarily individuals who require assistance due to post-operative recovery, chronic conditions, or age-related disabilities. You will have the opportunity to work closely with patients in their own environments, making a significant impact on their rehabilitation journey and overall well-being.

Qualifications
  • A Doctorate in Physical Therapy (DPT) from an accredited program.
  • Current Physical Therapy license as required by the state of Florida.
  • A valid CPR certification to ensure patient safety during therapy sessions.
Preferred Experience
  • A minimum of two years of outpatient orthopedic experience is required, providing exposure to a variety of conditions and treatment methodologies.
  • Proven ability to design and implement effective treatment plans that cater to individual patient needs.
Compensation and Benefits

This full-time position offers a competitive salary range of USD 100,000 – 120,000 annually, commensurate with experience. In addition to a comprehensive salary, we provide a robust benefits package, including:
