Position Overview

Swift Therapy Center is actively seeking a dedicated and skilled Physical Therapist (PT) to join our dynamic team in Jacksonville, Florida. Work Environment

Your work setting will be in our outpatient office, where you will benefit from a state-of-the-art facility equipped with the latest therapy tools and technologies. As part of a supportive interdisciplinary team, you will collaborate with other healthcare professionals, including physicians and occupational therapists, to provide comprehensive and personalized care. The standard working hours are Monday through Friday from 8 AM to 5 PM, offering a consistent schedule that promotes work-life balance.

Key Responsibilities
  • Conduct thorough consultations with patients to assess their needs, including reviewing medical histories and performing necessary assessments.
  • Design and implement tailored treatment protocols, adjusting them as needed based on patient progress and response to treatment.
  • Document evaluations, treatment plans, and progress notes accurately and in a timely manner to ensure continuity of care.
  • Participate in discharge planning by coordinating follow-up care that aligns with patient-specific rehabilitation goals.
  • Maintain compliance with state, local, and federal regulations in all aspects of patient care and documentation.
  • Provide guidance and support to patients' families regarding care procedures and aftercare plans.
  • Actively engage in professional development through participation in team meetings and training sessions, sharing insights to enhance team collaboration.
Patient Population / Client Focus

In this role, you will primarily work with a diverse orthopedic patient population. This includes individuals recovering from surgeries, sports injuries, and chronic conditions requiring rehabilitation. Your expertise will empower patients to regain strength, mobility, and independence. By building strong therapeutic relationships, you will inspire trust and motivate patients throughout their recovery journey.

Qualifications
  • A Bachelor’s Degree in Physical Therapy is required; a Doctorate in Physical Therapy (DPT) is preferred.
  • Must possess a valid Physical Therapy license in the state of Florida.
  • Current certification in Basic Life Support (BLS) from the American Heart Association is mandatory.
  • Strong communication and interpersonal skills to effectively engage with patients, caregivers, and the healthcare team.
Preferred Experience
  • A minimum of 2 years of experience as a Physical Therapist, particularly in an orthopedic outpatient setting.
  • Demonstrated ability to create and implement effective treatment plans based on individual patient needs.
  • Familiarity with electronic medical records (EMR) systems for documenting patient interactions and treatment plans.
Compensation and Benefits
