Job Title: Home Health Speech-Language Pathologist – SLP

Location: Dublin, VA & Surrounding Areas (In-Home Patient Care)

Schedule: Full-Time / Part-Time / PRN (Flexible Scheduling Available)

Pay: Competitive Rates (based on experience)


Job Overview:
We are currently seeking a compassionate and skilled Home Health Speech-Language Pathologist – SLP to provide personalized, one-on-one therapy in a home setting throughout Dublin and the surrounding areas. This position is ideal for a Speech-Language Pathologist who enjoys working independently while helping patients improve communication, swallowing, and cognitive function within their daily environment.

As a Home Health Speech-Language Pathologist – SLP, you will work directly with patients recovering from illness or injury while developing individualized treatment plans focused on improving patient outcomes and quality of life.


Responsibilities of Home Health Speech-Language Pathologist – SLP:

  • Provide home-based speech therapy to patients experiencing communication, swallowing, or cognitive difficulties
  • Conduct patient assessments and develop individualized treatment plans
  • Observe and evaluate a patient’s ability to communicate, swallow, and process information
  • Monitor patient progress and modify care plans as needed
  • Assist patients with mouth, breathing, swallowing, fluency, articulation, and reading exercises
  • Educate patients and families on therapy goals, safety practices, and self-care techniques
  • Collaborate with interdisciplinary team members including RN, LPN, CNA, HHA, PT, and OT
  • Maintain accurate and timely documentation of patient progress

Qualifications of Home Health Speech-Language Pathologist – SLP:

  • Master’s or Doctoral degree in Speech-Language Pathology
  • Active SLP license in the state of Virginia
  • Minimum of 2 years of speech therapy experience (home health preferred)
  • CPR certification and PPD/TB skin test required
  • Knowledge of state and federal home health regulations
  • Strong clinical judgment, critical thinking, and communication skills
  • Compassionate, dependable, and patient-focused
  • Ability to work independently in a home setting
