Mental Health Therapist (LSW/LPC)
Location:Cleveland, OH 44135
Shift: Full Time
About Stepping Stone
Stepping Stone is an outpatient addiction treatment program. We’ve designed this program to promote the self-reliance and confidence needed for individuals to integrate back into the community. Stepping Stone caters to patients who require continued physical rehabilitation or continued long-term care in a skilled care facility after leaving an acute care environment or hospital setting. Stepping Stones Behavioral Health offers Individual psychotherapy, Medication-assisted treatment, Case management, Peer support, and Telehealth treatment
The Role Itself
  • Conduct bio-psychosocial assessments under supervision to identify client needs.
  • Provide group counseling and teach effective coping strategies.
  • Develop treatment goals and individualized plans to address client issues.
  • Lead group and individual counseling sessions as needed, and participate in treatment team meetings.
  • Prepare detailed case summaries and written reports in accordance with program standards and professional ethics.
  • Ensure timely and accurate documentation that meets both facility and regulatory standards.
  • Facilitate safe and effective discharge planning, coordinating care with referral sources and community partners.
  • Demonstrate strong organizational skills and attention to detail while maintaining strict confidentiality.
  • Perform additional tasks as necessary to support the achievement of program objectives.
  • Assist with telehealth visits as required.
  • Complete all required documentation in the EMR system promptly.
  • Submit daily reports to track progress and outcomes.
Education and other requirements:
  • Must be 21yrs or older.
  • Must have a valid drivers license
  • Ohio Medicaid billing Number
  • NPI Number
  • Must have a minimum of an associates degree and Ohio License to practice.
