Job description

Join a growing home infusion pharmacy team and make a direct impact on patient care! We're seeking an experienced Clinical/Infusion Pharmacist for a full-time, direct hire opportunity in Columbus, OH. If you have a strong infusion background and enjoy collaborating with multidisciplinary teams, this could be the perfect fit.

Position Details
Location: Columbus, OH 
Job Type: Direct Hire
Start Date: ASAP
Schedule: Monday – Friday | 8:30 AM – 5:00 PM
On-Call: Participation in rotating on-call schedule required


Key Responsibilities
Prepare and verify sterile compounded IV medications in accordance with USP standards and regulatory guidelines
Monitor patient therapies, including pharmacokinetic dosing and TPN management
Review laboratory values and recommend clinical interventions when appropriate
Counsel patients and caregivers on infusion therapies, infusion pumps, catheter care, and home administration techniques
Collaborate with physicians, nurses, reimbursement specialists, intake teams, and pharmacy operations to ensure exceptional patient outcomes
Maintain compliance with Board of Pharmacy regulations, HIPAA, DEA, Medicare, ACHC, URAC, OSHA, and other applicable standards
Manage inventory, including refrigerated medications, and oversee safe medication preparation and delivery processes


Qualifications
Active Ohio Pharmacist License in good standing
2+ years of home infusion or specialty infusion pharmacy experience
Experience with sterile compounding and clinical monitoring
IVIG experience is a plus
Knowledge of specialty pharmacy reimbursement, payer requirements, and contract obligations preferred
Experience with CareTend is a plus
