Position Overview:
We are seeking a compassionate and experienced Palliative Care Nurse Practitioner or Physician Assistant to provide patient-centered care focused on symptom management, pain control, and quality of life for individuals with serious or chronic conditions.
This position supports inpatient palliative care, community-based care, and home visits, with flexibility to practice across settings based on patient needs and program demand.
Key Responsibilities:
โข Provide comprehensive palliative care consultations across inpatient, community, and home-based settings
โข Develop and manage inpatient consultation emphasizing comfort, symptom control, and patient goals
โข Collaborate closely with physicians, nurses, social workers, chaplains, and interdisciplinary care teams
โข Serve as a clinical leader within the interdisciplinary team
โข Educate patients and families on disease progression, treatment options, and goals of care
โข Demonstrate strong time management while managing a diverse patient caseload
โข Participate in interdisciplinary meetings, case conferences, and care planning
Qualifications:
โข Active California Nurse Practitioner or Physician Assistant license
โข Strong interdisciplinary team leadership skills
โข Excellent time management and organizational abilities
โข Comfortable with home visits and community driving
โข Strong communication skills with patients, families, and care teams
โข Ability to work independently while coordinating care across settings
Preferred / Plus Qualifications:
โข Credentialed at either Orange County, LA County, Long Beach, Temecula Valley, Riverside, San Bernardino, San Diego County area hospitals
