Key Responsibilities
  • Conduct thorough assessments, providing counseling, therapy, and psychotherapy tailored to individual client needs.
  • Develop and implement comprehensive treatment and discharge plans that align with client goals and best practices.
  • Utilize interviews, case history reviews, and observational techniques to gather detailed patient data, leading to informed evaluations and effective therapeutic approaches.
  • Maintain a diverse caseload involving both individual and collateral therapy, ensuring personalized care strategies for each client.
  • Conduct regular reviews of client case files for quality assurance, ensuring compliance with timeliness and accuracy in all documentation, including assessments, care plans, and progress notes.
  • Adapt assessment protocols according to health plan regulations and emerging best practices to ensure the highest standard of care.
Patient Population / Client Focus

You will work primarily with patients in hospice care, facing complex end-of-life issues and a range of emotional challenges. This role requires a deep empathy and understanding of the processes individuals undergo when navigating terminal illnesses, emphasizing the importance of emotional support and guidance during such critical times. Your efforts will directly contribute to enhancing the quality of life for patients and families, helping them find hope and peace despite daunting realities.

Qualifications
  • A Master's Degree in Social Work from an accredited institution.
  • An active and unrestricted Clinical Independent Social Worker License (LICSW).
  • A minimum of 2 years of relevant experience in a healthcare setting, with at least one year in a mental health environment.
  • Proven clinical experience in case management or as a social worker, demonstrating a compassionate and client-focused approach.
  • Exceptional conflict management and problem-solving skills, essential for supporting patients and families in sensitive situations.
Preferred Experience
  • Experience working specifically within hospice or palliative care, providing a nuanced understanding of end-of-life issues and family dynamics.
  • Familiarity with electronic health record systems and related documentation processes.
  • Certifications in specialized therapeutic techniques or methodologies, enhancing your ability to meet diverse patient needs.
Compensation and Benefits
