Work Environment

This role offers the unique opportunity to work remotely while impacting the lives of clients directly. You will be operating within our innovative Hospice Center, utilizing technology to connect with clients and team members. Key Responsibilities

• Conduct thorough assessments to identify clients’ unique needs, encompassing social, emotional, and environmental challenges.
• Develop individualized service plans that outline specific goals, interventions, and methods for tracking progress, collaborating closely with clients to ensure their voices are heard.
• Provide direct counseling and emotional support to clients facing difficulties, ranging from crises to ongoing life challenges.
• Engage clients in actionable steps towards their goals, connecting them with necessary community resources and support networks.
• Implement crisis intervention strategies, including risk assessments and safety planning, to manage immediate concerns effectively.
• Facilitate collaboration with community agencies and organizations to amplify resources and advocacy efforts for clients.
• Participate in community outreach initiatives to raise awareness around the services we provide and invite potential clients to access the assistance they need.
• Contribute to team discussions, case conferences, and ongoing staff training, enhancing both personal and team growth.

Patient Population / Client Focus

In this role, you will primarily work with a diverse client base that includes individuals facing significant life transitions, chronic illness, or end-of-life issues within a hospice setting. You will provide much-needed support to clients and their families, helping them navigate complex emotional landscapes while ensuring they receive compassionate and appropriate care. Your interventions will focus on enhancing their quality of life and dignity by providing holistic and personalized support systems.

Qualifications

• A Master's degree in Social Work from an accredited institution is required.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Florida is essential.
• Strong understanding of social and economic justice issues relevant to our target population.
• Knowledge of trauma-informed care principles and effective communication skills to engage diverse client demographics.
• Proficient problem-solving abilities and cultural competence are critical to positively impact client interactions.

Preferred Experience

• Prior experience working in a social service agency or similar environment is highly desirable.
• Familiarity with community resources and advocacy models that enhance client outcomes is preferred.
• Experience in providing support in palliative or end-of-life care settings will be an advantage.

Compensation and Benefits

This full-time position offers a competitive salary ranging from USD 60,000 to USD 100,000 annually, commensurate with experience and qualifications. Additionally, a sign-on bonus is included as part of the employment offer.
