Job description

Referral Specilalist
At Project HOME, every role contributes to our mission of empowering individuals to achieve stability, wellness, and self-determination. As a Referral Specialist, you'll help patients navigate the healthcare system by coordinating referrals, connecting them with specialty care, and ensuring they receive the services they need without unnecessary barriers.
This role is ideal for someone who is highly organized, detail-oriented, and comfortable independently managing a high volume of referrals. You'll work closely with providers, patients, hospitals, and specialty practices to move referrals forward, track progress, and help ensure continuity of care.
If you have healthcare referral experience, are comfortable navigating EHR systems, and enjoy taking ownership of detailed work that directly impacts patient care, we'd love to hear from you.
Location: Stephen Klein Wellness Center, 2144 Cecil B. Moore Avenue, Philadelphia, PA 19121
Schedule: Monday-Friday | 9:00 AM-5:00 PM
Compensation: $21.65 - $25.46/hr (based on experience)
What You'll Do
  • Manage referrals from provider order through completion of specialty care.
  • Schedule referral appointments and provide follow-up instructions.
  • Assist uninsured patients with insurance applications and financial assistance.
  • Track hospital admissions and support continuity of care.
  • Provide front desk coverage and schedule appointments.
  • Maintain accurate documentation and collaborate with care teams.

What You'll Need
  • High school diploma or GED.
  • Two years of experience in a similar role.
  • Experience using an EHR, Practice Management System, or EDR.
  • Strong organizational, communication, customer service, and computer skills.
  • Ability to work independently and collaboratively.

What You May Bring
  • Bilingual (Spanish/English).
  • Experience in an FQHC or community health center.
  • Knowledge of Pennsylvania COMPASS.
  • Experience serving vulnerable populations.
