Family Physician - Per Diem
Pay Rate: $105/hour
Job Summary: The Family Physician - Per Diem is a key member of an integrated health care team and instrumental to its success. This position provides primary medical care services and health education/screening activities to meet the healthcare needs of the Project HOME community, Project HOME residents, neighbors in the community, homeless persons & staff. Per Diem Physicians can work anywhere between 4-30 hours, based on availability, during the various sessions Monday-Friday. Sessions are 8:30am-12:30pm and 12:30pm-4:30pm.

As a Family Physician - Per Diem, you'll join a collaborative, mission-driven care team providing comprehensive services to individuals experiencing homelessness, underserved communities, and Project HOME residents and staff.

Your Impact
In this role, you will:
  • Provide primary care services for children, adults, and pregnant patients
  • Perform basic office procedures and LARC placement
  • Collaborate across interdisciplinary teams to address clinical and social needs
  • Support care coordination, patient education, and community health initiatives
  • Precept and mentor learners in a dynamic clinical setting
  • Contribute to a culture of compassion, respect, and high-quality care
What You Bring
Required:
  • Completion of Family Medicine residency
  • Board Eligible or Board Certified
  • Current CPR certification
Preferred:
  • At least 1 year of primary care experience
  • Experience in underserved settings (FQHC, free clinic, etc.)
  • Comfort with OB/GYN care
  • Strong collaboration, communication, and organizational skills
