Pro Active Physio Clinic is seeking dedicated and skilled Physical Therapists (PT) to join our dynamic team in Sunnyvale, California. This full-time position requires a commitment to providing high-quality patient care and rehabilitation services in our onsite rehabilitation center. Key Responsibilities

• Conduct thorough evaluations of patients to assess their physical capabilities and limitations, ensuring a comprehensive understanding of their needs.
• Design and implement individualized treatment plans based on evaluations, collaborating with patients to set achievable goals and expectations.
• Continuously monitor and reassess patients throughout their treatment process to adapt goals and interventions according to their progress.
• Maintain accurate and timely documentation, including SOAP notes, progress reports, and care plans using our electronic record system.
• Educate patients and their families about the rehabilitation process, addressing concerns and developing strategies for overcoming barriers during recovery.
• Meet or exceed utilization goals and participate in team objectives to enhance clinic operations.
• Work with other staff members across disciplines to coordinate care for patients receiving multiple therapeutic services.
• Exemplify and uphold the core values of Pro Active Physio Clinic, acting as a brand ambassador in all interactions.

Patient Population / Client Focus

Qualifications

• Must hold a Master's degree or a Doctor of Physical Therapy (DPT) from an accredited physical therapy program, ensuring a strong foundational knowledge of therapeutic practices.
• A current and unrestricted license to practice physical therapy in the state of California, affirming compliance with state regulations and standards.
• Willingness to perform evaluations and treatments both in-person and via telehealth when appropriate, aligning with modern healthcare solutions.
• Demonstrated commitment to working with a broad range of patients, utilizing culturally competent practices in physical therapy.
• Ability to thrive in a fast-paced, dynamic environment, displaying flexibility and adaptability in all situations.
• Strong interpersonal skills with a focus on collaboration and customer service, fostering positive relationships with patients and colleagues alike.

Preferred Experience

• Previous experience in a rehabilitation setting, particularly working with a multidisciplinary team, is highly desirable.
• Familiarity with electronic medical records (EMR) systems and the ability to navigate digital documentation efficiently.
• Specialized training or certifications in particular areas of physical therapy practice may be advantageous.

Compensation and Benefits

The annual salary for this position ranges between USD 100,000 and 120,000, commensurate with experience and qualifications. Additionally, employees receive a comprehensive benefits package that includes:
