Monday – Friday

8:00am – 4:30pm

REQUIREMENTS:

  1. Board Certified or Board Eligible in Family Practice or Internal Medicine.
  2. Accredited Doctoral Degree.
  3. Basic Life Support (BLS) certification.
  4. Drug Enforcement Agency (DEA) registration.
  5. Professional License to practice medicine.
  6. Proof of experience is required to be within the past 5 years.
