Responsibilities:
The Care Coordinator will support the Valencia and Socorro, New Mexico areas. The role facilitates a team-based approach, including the Interdisciplinary Care Plan (ICP) team, to ensure appropriate interventions and the cost-effective delivery of high-quality care and services across the continuum of care.
The Care Coordinator collaborates with the interdisciplinary care plan team, which may include the member, caregivers, the member's legal representative, physicians, care providers, and ancillary support services, to address care issues, specific member needs, and disease processes. These may include medical, behavioral, social, community-based, and long-term care services.
The Care Coordinator is responsible for coordinating care for individual clients and identified populations through comprehensive assessment, care planning, implementation, coordination, monitoring, and evaluation to ensure cost-effective, quality outcomes.
Some key responsibilities include
Supports health plan members through ongoing care coordination.
Facilitates a team-based approach with the Interdisciplinary Care Plan (ICP) team to ensure appropriate, cost-effective, high-quality care across the continuum.
Collaborates with members, caregivers, providers, and ancillary services to address medical, behavioral, social, and long-term care needs.
Conducts comprehensive health risk and needs assessments, including psychosocial, medical, behavioral, environmental, and financial factors.
Develops, implements, and communicates individualized care plans, including service authorizations and transitions of care.
Coordinates and monitors care plans to identify gaps, improve outcomes, and ensure quality and cost-effectiveness.
Implements and evaluates interventions to improve members' health outcomes and quality of life.
Serves as an advocate for members by identifying and addressing gaps in care and access to services.
Provides education and support to members, families, providers, and staff regarding care coordination and health management.
Maintains effective relationships with internal teams and external stakeholders, including community resources and healthcare providers.
Qualifications:
- Masters Degree & 1 years exp, Bachelors Degree & 2 years exp, Associates Degree & 3 years exp, 6 years of exp may be utilized in lieu of other education
- Must have a valid driver license, clean driving record, and able to travel locally.
- Experience in utilization management, quality assurance, home care, community health, long term care or occupational health required. CCM certification preferred or must obtain within 3 years of hire.
- Proficiency in Microsoft Word, Excel and Outlook required. Experience in analyzing trends based on decision support systems.
- Business management skills to include, but not limited to, cost/benefit analysis, negotiation, and cost containment.
- Knowledge of referral coordination to community & private/public resources.
