About the Role

As a Qualified Intellectual Disability Professional (QIDP) at Pathfinder, you'll help ensure the people we support have individualized plans, meaningful goals, and the support they need to live, learn, and work successfully as active members of their communities. You'll work closely with individuals, families, Direct Support Professionals, Group Home Managers, and other members of the Interdisciplinary Team to make sure services are person-centered, consistent, and focused on what matters most to each person. 

As a Qualified Intellectual Disability Professional (QIDP) at Pathfinder, You Will:

Person-Centered Planning & Support

• Coordinate and oversee Individual Support Plans (ISPs) to ensure they are person-centered and reflect each individual's wants, needs, and goals. 
• Use person-centered thinking skills and tools in conversations, team meetings, planning, and daily interactions. 
• Work with individuals and their Interdisciplinary Teams (IDTs) to develop meaningful objectives and behavior plans. 
• Monitor progress toward goals and objectives and make revisions when appropriate. 
• Arrange services and supports identified through the person-centered planning process. 
• Promote informed choice, quality of life, independence, and empowerment. 

Behavior Support & Individual Assessments

• Complete assessments related to individual functioning and behavioral needs. 
• Develop appropriate Behavior Support Plans (BSPs) when needed. 
• Seek Human Rights Committee (HRC) approval for plans requiring review. 
• Monitor implementation of behavior plans and provide direction to staff to support consistent implementation. 
• Provide leadership and training related to crisis prevention, intervention, teaching skills, and person-centered approaches. 

Team Leadership & Training

• Provide leadership, direction, mentoring, and training to Direct Support Professionals and others. 
• Train staff on ISP objectives, behavior plans, personal assistance, residential habilitation, prevention of abuse and neglect, and person-centered planning. 
• Work alongside DSPs in group homes to demonstrate and reinforce person-centered support practices. 
• Collaborate closely with Group Home Managers to ensure information is communicated consistently to house staff and IDT members. 
• Serve as a primary contact for IDT members, including guardians, families, case managers/behavior consultants, job coaches, and other partners. 

Communication & Collaboration

• Participate in person-centered planning and ISP meetings with individuals, families, staff, and community partners. 
• Facilitate, scribe, take notes, and assist with planning during meetings as needed. 
• Maintain regular communication with individuals supported, employees, families, guardians, and other members of the IDT. 
• Build positive, collaborative relationships with coworkers and Pathfinder leadership. 
• Promote open communication so everyone involved in supporting an individual understands their role. 

Documentation & Compliance

• Maintain accurate group home files electronically in Provide and in paper form, as needed. 
• Schedule annual case conferences and quarterly meetings while ensuring individuals have a voice in how and with whom meetings are conducted. 
• Maintain documentation demonstrating that ISPs and BSPs are being followed. 
• Ensure compliance with ISDH regulations, federal and state guidelines, including Title 460 of the Indiana Administrative Code. 
• Participate in Human Rights Committee activities and prepare and present reports for plans requiring HRC approval. 

Direct Support & Residential Services

• Maintain an ongoing, flexible schedule that includes direct support in assigned group homes. 
• Assist with medical appointments and transportation as needed. 
• Work directly alongside DSPs to mentor, demonstrate, and educate staff on person-centered approaches. 
• Help staff properly implement individual goals, objectives, and Behavior Support Plans. 
• For assigned homes, provide approximately five hours of direct care per week in each home when assigned more than two homes, or ten hours per week when assigned two homes or fewer. 
• Coordinate direct care with each home's manager and vary hours to include mornings, evenings, and weekends as needed. 

Quality, Resources & Continuous Improvement

• Promote the efficient use of organizational resources, including human and financial resources. 
• Cooperate with internal and external auditors by providing documentation and timely responses. 
• Assist with developing and implementing corrective action plans related to audit findings. 
• Review processes and procedures and recommend adjustments that improve services and supports. 
• Participate in professional development opportunities to strengthen knowledge and skills.

Requirements

Preferred Minimum Qualifications

• Bachelor's degree required in a developmental human service or therapy field.
• Minimum of two years of experience in the Human Service field.
• Proven team management skills.
• Ability to interpret, or learn to interpret, financials.
• Goal driven. 
• Positive attitude and team player who welcomes collaborative decision making. 
• Ability to work a flexible schedule, if needed.
• Crisis prevention, intervention, and teaching skills.
• Valid driver's license and current auto insurance required.
• Ability to speak, hear, and read the English language.
• The QIDP shall have essential physical skills: be able to run, squat, stoop/bend, kneel, climb stairs, lift up to 50 pounds, push and pull against potential resistance, and implement non-physical and approved physical interventions and CPR with people supported when needed. They shall have adequate vision, writing, and documentation skills to perform the essential functions of the job.
