Work Environment

This role is designed for a full-time licensed Clinical Social Worker who will operate remotely, primarily supporting clients in a school setting. The work schedule from Monday to Friday, 8:00 AM to 4:30 PM, allows for regular office hours with occasional flexibility required based on clients' needs. Evening or weekend hours may be necessitated to ensure continuous care and client access. This position requires excellent communication skills as you'll collaborate closely with various stakeholders, including educators, families, and community organizations.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify the emotional, mental, and social needs of senior clients and their caregivers.
  • Develop and implement personalized care plans that reflect each client's goals, preferences, and specific challenges, using evidence-based practices.
  • Provide counseling and emotional support to seniors and their families, assisting them through life transitions such as bereavement, retirement, or health changes.
  • Serve as an advocate for clients, facilitating access to community resources and services that cater to their needs, including healthcare, housing, and social support.
  • Maintain meticulous, confidential records of client interactions and treatment plans in compliance with professional standards and regulations.
  • Engage in ongoing communication with multidisciplinary teams to discuss client progress and integrate feedback into care adjustments.
Patient Population / Client Focus

This position focuses on serving seniors who often grapple with a myriad of social and psychological issues, including loneliness, mobility limitations, cognitive decline, and health management. Your role will be critical in providing emotional support, promoting well-being, and advocating for their rights and needs within the community. By working directly with seniors and their families, you will have the opportunity to forge meaningful connections and facilitate access to essential services.

Qualifications
  • Master's degree in Social Work from an accredited institution.
  • Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
  • A minimum of two years of hands-on experience in clinical social work, ideally in a geriatric or community setting.
  • Thorough understanding of social work principles, particularly as they relate to aging populations, mental health, and the various components of elderly care.
  • Exceptional interpersonal and communication skills, enabling effective collaboration with diverse clients, including seniors, families, and cross-disciplinary teams.
  • Proficient in clinical assessment and counseling skills, with a demonstrated ability to engage clients holistically.
Preferred Experience
  • Experience working within a school setting or collaborative care models is highly desirable.
  • Knowledge of community resources, programs, and services specific to the needs of seniors.
  • Proficient with electronic health record (EHR) systems and telehealth technology.
Compensation and Benefits
