PROMIS Neurodevelopmental is seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our team in a remote capacity, supporting clients within our hospice care setting. Work Environment

This full-time position operates remotely, allowing for a flexible work-life balance while providing vital services to our clients. While the role is conducted from home, you will be an integral part of a multidisciplinary team, regularly interacting with medical providers and case managers via telehealth platforms and virtual meetings. Key Responsibilities
  • Conduct individual, group, and family therapy sessions using culturally competent and evidence-based practices tailored to meet diverse client needs.
  • Perform comprehensive biopsychosocial assessments to gain a holistic understanding of client challenges and develop personalized treatment plans.
  • Engage in close collaboration with a multidisciplinary team, including medical personnel, nursing staff, case managers, and peer support specialists, to ensure integrated service delivery.
  • Facilitate connections to community resources, providing clients and their families with vital support services, especially during crises or transitions.
  • Ensure thorough and up-to-date clinical documentation that adheres to ethical standards and complies with legal requirements, enhancing both quality assurance and client care.
  • Actively participate in team meetings, case conferences, and ongoing professional development opportunities to stay apprised of best practices and new methodologies in social work.
Patient Population / Client Focus

The hospice care environment at PROMIS Neurodevelopmental requires sensitivity and expertise in managing the unique needs of clients facing end-of-life issues, including mental health concerns, grief, and trauma. We serve a diverse population of adolescents, adults, and families, focusing on their psychological well-being while addressing the complex emotional and psychosocial aspects of chronic illnesses and terminal conditions.

Qualifications
  • Possession of an active LCSW license in the state of California issued by the Board of Behavioral Sciences.
  • A Master’s degree in Social Work (MSW) from an accredited institution is mandatory.
  • Minimum of 2 years of post-licensure experience is required to ensure clinical competency.
  • Excellent clinical assessment and intervention skills, with a strong emphasis on empathetic client engagement.
  • A background in working with children and youth is highly preferred, reflecting our commitment to serving all ages.
Preferred Experience
  • Previous experience in hospice, palliative care, or similar settings where trauma-informed care is prioritized.
  • Familiarity with community resources and their application in a clinical context.
  • Proficiency in telehealth delivery and remote client relationship management.
Compensation and Benefits
