Job Summary:
Providence Napa Home Health is seeking an experienced and motivated Home Health Physical Therapist to join our dedicated rehabilitation team. The PT will provide skilled physical therapy services in patients' homes across Napa, CA, and surrounding areas. This is an autonomous role with a manageable caseload and excellent support from a collaborative care team.
Responsibilities:
  • Deliver physical therapy services to home health patients in accordance with individualized care plans.
  • Perform patient assessments, develop treatment goals, and monitor progress.
  • Document visits, outcomes, and reassessments using EPIC and OASIS systems.
  • Travel to patient homes within the designated territory (occasional travel radius up to 50+ miles).
  • Maintain a standard productivity expectation of 30 points/day (typically 5 6 patient visits).
  • Collaborate with interdisciplinary team members to support patients' overall care plans.
  • Participate in required documentation, meetings, and compliance training.
Requirements:
  • Minimum 1 year of recent skilled Home Health Physical Therapy experience.
  • OASIS experience required.
  • Active BLS Certification.
  • Must have valid driver's license and reliable transportation.
  • Must be COVID vaccinated (primary series + 1 booster required).
  • Pending CA license accepted; must be active prior to start.
  • Willing to work 1 weekend shift per contract.
  • Open to local candidates (within 50 miles) at same rate.
