As a Patient Service Navigator for Hospice, you will play a key role in supporting patients, families, providers, and clinical staff by coordinating communication, scheduling, referrals, and documentation in a fast-paced office environment. This position serves as an important first point of contact and helps ensure compassionate, efficient care delivery.

What You’ll Do:

  • Serve as the first point of contact for patients, families, and providers—answering calls, handling referrals, and communicating directly with patients.
  • Manage scheduling of clinical staff visits in a busy office environment.
  • Ensure appropriate assignment of referrals in consultation with supervisors.
  • Assist in the development, establishment, and evaluation of team documentation systems in conjunction with Supervisor to assure accuracy, timeliness, and efficiency.
  • Maintain up-to-date knowledge base on all insurance forms, chart forms, insurance paperwork requirements, and share information with the team routinely.
  • Establish priorities for team paperwork to be done on a daily/weekly/monthly basis.
  • Review all paperwork for completeness of forms on a regular basis.
  • Develop and maintain accurate systems to ensure timely coordination of all multi-discipline documentation.
  • Perform data entry into Electronic Health Record (EHR) of some OASIS & staff timesheets.
  • Maintain daily/holiday/weekend schedules.


What You’ll Bring:

  • High School diploma required.
  • Prior office or healthcare scheduling experience strongly preferred.
  • Electronic Medical Records experience a plus!
  • Strong, demonstrated computer skills with comprehensive knowledge of Microsoft Word, Excel, Outlook, and PowerPoint.
  • Excellent customer service and public relation skills with a friendly, outgoing, and professional demeanor.
  • Ability to thrive in a fast-paced, team-oriented environment while also being self-sufficient.
  • Task-oriented with strong multitasking skills to manage scheduling, referrals, and patient communications simultaneously.
  • Organizational skills with close attention to detail
